What Actually Happens During a Counselling Session?

For someone who has never experienced counselling before, the uncertainty surrounding the first appointment can sometimes be enough to delay making contact. People may wonder what they are expected to say, whether they will be asked uncomfortable questions or if they need to explain their entire life story immediately. In reality, counselling is usually a much more collaborative process, and the first session is an opportunity for both the client and counsellor to begin understanding what has brought the person there.

An initial counselling session will often include some discussion about how the service works. This may cover confidentiality, appointment arrangements and the boundaries of the therapeutic relationship. The counsellor may ask what prompted the client to seek support and what they hope might be different as a result of counselling. There is no need to have a polished answer. It is completely reasonable to begin with something as simple as knowing that life has recently become harder.

The conversation will then develop according to the issues the client wants to explore. Some people arrive with a specific concern such as anxiety, bereavement, a relationship breakdown or stress at work. Others have several connected difficulties or a general feeling that something is not right. A counsellor can help organise these experiences into themes without forcing the conversation in a direction the client is not ready to take.

Counselling is also different from receiving advice from a friend. A counsellor will not normally tell someone exactly what decision to make. Instead, questions and discussion can help the client understand their own reactions, assumptions, emotional triggers and choices. This can be particularly valuable when someone has been thinking about the same problem for a long time and feels unable to see it from another perspective.

Not every session will feel dramatic. Some appointments may involve important realisations, while others may be quieter and focused on recent events or gradual progress. The therapeutic process develops over time, and clients should be able to discuss whether the approach feels useful and what they would like to focus on.

Good counselling services should provide a setting in which people feel respected and able to speak honestly at their own pace. The first appointment is therefore not a test and there is no correct way to “do” counselling. It is simply the beginning of a professional conversation designed to help someone understand what they are experiencing and identify ways of moving forward.
